Patient Portal:
Our patient portal remains available only for the purpose of accessing your medical records. Since our practice closure on March 30th 2024, we can no longer receive any messages through this portal.

The web portal allows patients to perform the following activities:
- View previous lab results. To see your labs, click on the lab/diagnostic imaging tab. Your labs will be listed individually by date. Just click on the name of the lab to see the value, a normal range and any notes your physician may have written. You may also print a copy of your results for your records.
- Scanned documents can also be viewed and downloaded through the Documents tile on your Dashboard. These documents would have to be manually published by your provider or staff member in order for you to view them.
- The link to download your requested medical records is embedded in a message from the practice. Click on the Messages tile on your Dashboard to view this. Please make sure to access the records as soon as you receive the notification as the link expires after 7 days.
