Dear Patients and Families,

I regret to inform you that Endocrine and Diabetes Associates, LLC will permanently close its doors on March 31, 2024. As a partner at the practice for over 20 years, I cherish the meaningful associations with all of you and thank you for enriching my life.

I have always found my clinical work to be consequential and fulfilling, and now I aim to evolve this chapter into a fresh and enthusiastic beginning. I've launched my own solo practice called Advanced Endocrinology, LLC. I plan to start providing services in my new office as early as April 1, 2024, ensuring continuity of care. I'm in the process of credentialing the new practice with various payors to continue accepting insurance reimbursements, avoiding undue financial burdens on all of you.

In addition to caring for all my existing patients at the new location, I'll be seeing new patients and striving to meet the growing need for endocrine care in the community. Advanced Endocrinology, LLC will be located at 15200 Shady Grove Road, Suite 202, Rockville, MD 20850, with easy access from I-270 and plenty of free parking.

You may also visit our website (advancedendo.org) for informational purposes. It will be fully functional with eCW and Healow Portal and Appointment Access at least one week prior to our opening. After April 1, you can also reach us at 301-297-3330 by phone/text or email us at aestaff@advancedendo.org.

Until the end of March 2024, I'll be available at our Bethesda office. However, my schedule is currently full, and I can only accommodate a limited number of appointments until April 1, 2024. I'll continue to access our Electronic Medical Record system at Endocrine and Diabetes Associates, providing prescription refills and diabetic supplies orders for my patients for four weeks after the closing to avoid any treatment disruptions.

If you already have an appointment scheduled with me on or after April 1, please note that you will be seen at Advanced Endocrinology in Rockville.

Established patients seen by me at least once in the last 5 years won't need to request their medical records, as I've arranged for them to transfer from our current practice to Advanced Endocrinology, LLC. All electronic medical records from Endocrine and Diabetes Associates will be sent to a medical records custodian, and you'll have access to your health care records directly. Further information regarding the medical records custodian will be available in the forthcoming weeks at https://www.endocrinemd.net.

I look forward to our continued association and express my immense gratitude for the privilege of being a partner in your journey to better health.

Best Wishes,

Anurag Gupta, MD
